Ordering production materials

Materials are to be ordered by means of the LASAK CadCam order form. The order, made by means of the order form and then acknowledged, is considered as being a valid order for both parties.

Pricing

The prices for the materials ordered are in accordance with the prices shown in the LASAK CadCam price list, valid at the date of the order. All prices shown in the price list do not include any commission, VAT or other duties, nor transport, postage or packing costs.

Transportation

The transportation costs depend on the destination. The buyer will be provided with the information related to the transportation costs.

Shipment insurance

Shipments are insured by the vendor against loss or damage during transportation, provided that the recommended mode of transport is used. If a different mode of transport is used, the insurance premiums are charged separately. The risk is transferred to the buyer as soon as the buyer has received the shipment.

Exchange/return of material

The buyer cannot withdraw from the contract if the goods delivered have been modified according to the buyer’s wishes (custom-made goods).

Payment terms

Prices are net for LASAK’s account. Delivery of material is based on invoicing with a due date 14 days from the date of issue. The invoice date being the date of dispatch is considered a standard payment condition. If the payment of the invoice is delayed, LASAK will have the right to change the payment condition to deliveries made on a cash-on-delivery basis, or to payment in advance. If payment is late, the buyer will be charged late payment interest equal to 0.1 % per day.

Protection of personal data and the use of cookies

By submitting the order form, the buyer agrees that the data provided may be processed (until permission is withdrawn) by LASAK s.r.o., company ID: 44265786, registered address: Ceskobrodska 1047/46, Prague 9 – Hloubetin as the administrator of the website, in compliance with Act No. 101/2000 Coll., on the protection of personal data, as amended. By submitting the order form, the buyer agrees that their personal data may be processed by LASAK using electronic means according to the provisions of Act No. 480/2004 Coll., on third party company information services, as amended. This permission is provided voluntarily and may, at any time and at no cost, be withdrawn by writing to: info@lasak.com or by means of a letter sent to: LASAK s.r.o., Ceskobrodska 1047/46, 190 00 Prague 9 – Hloubetin, Czech Republic. The personal data provided to LASAK s.r.o. may only be passed onto third parties for the purpose of dispatching shipments through a shipping company. The website may use technical information obtained through cookies. It is possible to disable cookies in browser settings. However, if the use of cookies is disabled, the website may not fully function. No personal data is obtained using cookies.

Guarantee terms and conditions

LASAK CadCam frameworks are manufactured from biocompatible, certified materials. LASAK hereby guarantees that, in case the manufacturer of the implant, as a third party, refuses implant replacement based on an alleged failure due to the use of LASAK individual milled solutions using a CAD/CAM method, within 10 years after the implant insertion, LASAK will provide the replacement implant and the superstructure free of charge. LASAK hereby guarantees that, in the case of a strength and stability failure of the LASAK product material within the guarantee periods (10-year guarantee on superstructures made of titanium or Co-Cr alloy, 5-year guarantee on superstructures made of the material ZrO2), LASAK will replace the product with the same or a substantially equivalent product. Guarantee terms are described in the LASAK Guarantee.

Final provisions

These general terms and conditions are only applicable to cases where the person intending to buy the goods from LASAK is a legal person, or the person in charge of ordering the goods within their business activities or within their business responsibilities. Relationships between LASAK and the buyer, who is also the consumer, are governed by the relevant provisions of the Civil Code of Czech Republic.
If any provision of the general terms and conditions is found to be invalid or ineffective, or becomes invalid or ineffective, a new provision will be agreed between the parties, which will be the closest possible approximation to the invalid clause. The invalidity or ineffectiveness of a single provision is without prejudice to the validity of the effectiveness of the other provisions. Any changes or additions to the purchase agreement, or the general terms and conditions, shall be made in writing.
By submitting the order form, the buyer declares their explicit consent to the content of these general terms and conditions applicable at the date of dispatch of the order to LASAK, including the price.
